knee extension machine??

Excuse my ignorance, but what exactly is a knee extension machine?

My story is so similiar to Nyer, its actually scarey. Same exact scenerio but I had surgery last June, still experiencing pain and have a hard time getting full ROM. When I wake in the am it is sometimes difficult to fully straighten the leg. It does not extend as fully as the good one (but I can hyperextend the good one - used to be able to hyperextend both). Joined a gym and go about 3x/week, continue with pt exercises but still have pain (also had tendonitis). What should I do (go back to surgeon or pt?)?
