I also went thru patella autograft and labored over same decision regarding leg extensions based on what I read on the internet and Board comments. But my PT indcated at time I stopped therapy and relying on the fitness center that I should be okay to do them at 4 months post-op. I decided to wait...but did many other excercises at the fitness center. Then at my 6 month post-op checkup, my OS did give me my sports release at about 85% full muscle strength but he encouraged me to continue with exercises and also told me to start the leg extensions. I told him my concerns - possible patella tendonitis and also if not careful how done could load up stress on the new ACL. He told me point blank to start them.
I did so but I followed the guidance that my PT had given me months earlier - set the ankle support higher off the ankle to minimize the lever arm (support to knee) and then to avoid the extreme positions of the exercise also not hoolding the extension but go thru the movement back to the beginning start point and repeat. Also...I started at very low resistances and once I could do 3 sets of about 15 repetitions each, with no ill effects a day or two later, then I increased the resistance level. I also did this as a single leg exercise.
I had absolutely no problems.....but years ago on this Board I received a number of EMails from those that had issues - In most cases, they started too early with the leg extensioons or went at them too aggressively. Then after that setback, they had to lay off of many exercises for 2 months or longer until tendonitis went away. In most cases, it goes away after a few months but in one or two cases, I and others have read about it taking ....years ! So....my advice if going this route, heed above cautions, go at it with slow resistance increases. The idea is to tire the muscles out with many reps at low resistance rather than use high resistances and risk straining or some other setback. Plenty of time to get to the higher resistance levels.
