Leff Knee Sprained Ligament

Three months ago I twisted my left knee. The doctor said it was a sprained ligament.
The MRI states:
"The examination shows a cortical thickening along the lateral side of the proximal fibula, probably related to an old healed fracture. No recent traumatic changes are seen in the knee joint, but there is some soft tissue swelling, probably fluid, in the suprapatellar both of..." (end of findings)
The treatment is conservative R.I.C.E.
Right now I am still having trouble walking. However, there has been slow but steady progress the last three months. Also, there is a very loud click on the left side of the knee when i straighten the leg. There is also pain around the kneecap.
Does anyone know if this condition is serious and require surgery? Any help of any kind would be appreciated.
