It depends on where the lesion is. In the patella, and rehab and suppliments are not sucessful, I do an anteriorization or anteromedialization of the tibial tubericle. In the medial or lateral joint compartments, if there is a deformity, I do an tibial or femoral osteotomy to correct the deformity. If there is no deformity, (which is the case most of the time), I just do rehab, and put the patients on chondral support medications.

I have not been convinced that mirofracture, or ACI is worth the trouble. I have only minimal experience with OATS, don't believe it is indicated for chondral disease of the patella.
