Re Tear of after Acl Reconstruction

My 15 year nephew tore his Acl in early Jun 2009.He had reconstruction at end of June 2009 with a patella graft (after deciding not to use cadaver). He rehab religiously also hired trainer to strengthening hamstrings etc..! He was released from the surgeon on Monday of this week (after 8 months) and 2nd day out at practice his reconstructed ACL re-tore !! Why would this happen??
