Dear Denise,

Has your granddaughter's knee been tested (via the standard manual-manipulation tests: Lachmann drawer test, anterior drawer test, pivot-shift test, Macintosh test, dial test, valgus and varus stress tests, posterior drawer test, and so on) by a knee-experienced doctor?

Please keep in mind that MRI scans sometimes miss ligament tears. For ligaments, MRI results should be combined with manual-manipulation testing...and if the manual-manipulation testing is inconclusive, then the testing should be repeated under anesthesia (to rule out residual muscle tenseness masking ligament deficiencies).

Also, the bruised femur is worrisome: this is articular-cartilage damage (bone bruising), which can be expected to bring premature osteoarthritis. Granted, there is generally little that can be done about such an injury other than to let it heal (via scarring-over) on its own. Usually, a protocol of avoidance of weight-bearing is recommended.

Yours truly,
Michael Frind.
