Michael,
Thank you for the post. I would say you hit it 100% on. I had surgery Feb 19 and have a new allograft ACL. The surgeon suggested the prior ACL wasn't placed well. When he went in for bucket handle tear on the meniscus, he said the ACL was so loose it wasn't functional. I suspect that it had loosened over time. In hindsight, I was avoiding using the leg as a base for pivoting (it can be whole foot or ball of foot pivoting) for the last 6-12 mos. I was doing 80% effort kicks when my Dec 2009 injury occurred (full effort requires a sharp snapping pivot). Meniscus wasn't repairable, so he cut it out, he said he removed about 30% of the meniscus. For the ACL, he had to widen two tunnels then he redid the shin tunnel so it was more lateral to provide more stability.
For anyone out there who has a bucket handle tear -- you are supposed to stop all activity until your surgery. It is very easy for the tear to worsen or the flap to get caught. I didn't know this and had been trying to build up muscle strength.
Dr recommended allograft for me because it was my 2nd surgery, I'm older (41), bad knees (they both ache) and I have tendency to build a lot of scar tissue.
I found it interesting I did not use a CPM machine the 2nd time around. I was at full extension by my 10 day postop. 70 degrees at 2 weeks post op, 100 degrees day 19. I had a nerve block 2nd time around which was heaven for about 24 hours, I highly recommend that. I did one percocet day 2 every 4 hours then a half of one day 3 then switched to Tylenol. I wasn't allowed to take ibuprofen until day 4 then added Alleve. Was too fuzzy to work at my computer until day 4. Crutches while brace locked at 0 (day 10), weaned off of them day 11. I can straight leg raise with no lag, but inside quad isn't really firing (doing elec stim for that).
Traveled via airplane 2 weeks postop, flight a little over an hour. I had an empty seat so I was able to put my leg up (and I'm short 5'1"). Got ice from restaurants and then from flight attendant.
Hope this helps someone out there!
