ABSTRACT
The
Lachman test has been recognized as one of the best clinical examinations for
detecting ACL deficiency.18 There has never, however, been a report in the
literature on voluntary anterior subluxation of the proximal tibia, where the
knee is maintained in the classic Lachman test position of slight knee flexion.
Main Text
Three
cases of voluntary anterior subluxation of the proximal tibia in an
ACL-deficient knee were identified at our institution. We used EMG to determine
that this phenomenon was produced primarily by contraction of the gastrocnemius
muscle. It is the purpose of this report not only to characterize this
interesting physical finding, but, more importantly, to describe the mechanism
of voluntary anterior tibial subluxation.
